Four Seasons Health Care is working in conjunction with the Belfast Health and Social Care Trust to relieve the winter pressures in A&E’s and decrease the number of complex delayed discharges from the acute hospital settings across the Belfast Trust Group of Hospitals and the Ulster Hospital Dundonald by providing a pilot of 35 short term beds across 6 sites in the Greater Belfast Area.
The aim of the service is to facilitate a care pathway for those patients who have been declared medically fit and no longer require treatment or a medical bed but who require a period of further social care assessment to ensure that their long term care needs are appropriately assessed.
The pilot consists of 25 2 week Interim Assessment beds in the following Homes- Domnall Intermediate Care Centre (8), Lansdowne Care Home (7), Bethany Care Home (4), Manor Lodge (2), Hawthorn House (2) and Strathearn Court Care Home (2) and 10 Fracture beds in Mount Lens Care Home.
To facilitate the day to day operational management and quantitative and qualitative reporting to evidence the effectiveness of the pilot, an Interim Assessment Bed Co- Coordinator has been appointed on a temporary basis for 5 months.
The Interim Assessment Bed Co- Coordinator role will
1. Support and report to the Regional Manager and Managing Director.
2. Provide communication between the Belfast Health and Social Care Trust Complex Discharge Co- Coordinating Team and the designated Home Managers.
3. Review and screen referrals in the absence of the Home Manager and carry out preadmission assessments at the request of /and in conjunction with the Home Managers.
4. Support the Home Manager role by quality assuring the admission documentation by completion of the Admission TRaCA.
5. Formulate a month end report to be shared with Regional Manager, Managing Director, Group Finance Director and Belfast Health and Social Care Trust. This month end summary will report on Bed Occupancy days by total 35 beds and by each home, average length of stay, readmission rates to hospital, patient focused outcomes and resident feedback reviews.
6. Participate in month end teleconferences with Regional Manager, Managing Director, Commercial Finance Manager and Belfast Health and Social Care Trust which will review bed occupancy days per home and work towards the flexibility of sites across the region as proposed and agreed with the Trust.
