Job Title:            Care Home Assistant Practitioner


Accountable to: Clinical Lead



The post holder will be expected to work flexibly across the Four Seasons Health

Care portfolio, providing support in homes throughout the United Kingdom for several weeks at a time.



As part of the Care Services Support Team, this role will provide assistance to a home that is in difficulty ensuring that residents are safe and receive a high standard of care; this role will provide support to Registered Nurse(s) to meet a range of clinical needs.


The role is an enhanced care assistant role that will provide clinical interventions to enhance the resident experience and the level of care residents receive under supervision of a registered nurse.


On a daily basis, support the nurse(s) to supervise and deliver excellent standards of care, which promotes privacy and dignity, whilst enhancing independence, rehabilitation and personal choice.  


Job Requirements


 Enhance resident experience:

  • Champions the Quality of Life programme, encouraging good levels of feedback from residents, relatives, visiting professionals and colleagues and assisting with the completion of audits using the iPad system.
  • Instil a culture of customer service
  • Undertakes appropriate training to contribute to the development of self and colleagues.
  • Performs escort duties as requested to hospital and other professional visits.  
  • Participates and supervises direct personal care promoting privacy and dignity at all times.

Supports nurses in leading the in-home clinical / care team:

  • Help encourage team working throughout the Home
  • Supports the nurse(s) in leading of the team and supervision of staff to ensure that shifts run smoothly, delegating as appropriate.
  • Takes the lead with ensuring that relevant supplementary documentation is completed by others in the team
  • Actively promotes team learning and development by encouraging team members to keep up to date with mandatory training and SOAR.

Perform clinical skills for which they have been assessed as competent

  • Will undertake clinical skills to include: physical observations/vital signs, urinalysis, skin inspection, aseptic technique including simple wound dressings, blood glucose monitoring and gastrostomy care and administration of gastrostomy tube feed
  • Recognise and act on changes in residents condition, including changes in physical and emotional health observations.
  • Undertake continence management
  • Oversee nutrition and hydration management
  • Fully understands and appreciates limitations of the role and knows when to get advice/seek guidance from the nurse in charge/home manager.

Performs medicine management within defined protocols

  • Undertakes designated aspects of medicine management in line with legal and company policy
  • Assists the nurse(s) plan and deliver appropriate end of life care
  • Assist the nurse(s) assessing and alleviating pain through both pharmacological and non-pharmacological means.
  • Safely orders, receives, records, stores and disposes of medications in line with legal and company policy
  • Liaising with the multi-disciplinary team in writing Best Interest decisions in relation to administration of covert medication under the supervision of the nurse(s)

Contribute to effective communication that ensures confidentiality

  • Good communication skills, including verbal and written skill
  • Assist in the admission, transfer and discharge of residents
  • Has knowledge and understanding of the care planning process
  • Ensures planned care is delivered in collaboration with residents, their family and members of the multi-disciplinary team.
  • Participate in the quality management system
  • Liaises appropriately with visitors and callers to the home referring them to senior staff as appropriate.
  • Records and passes on observations as required to maintain resident care and safety

Contribute to maintaining a safe environment

  • Has due consideration to the safety of others following legal, regulatory and best practice guidelines
  • assist in the reduction of infection risk
  • Demonstrates excellent standards of incident/accident and emergency situation management
  • contribute to a culture of safe moving and handling
  • recognise and resolve safeguarding risks
  • Maintains knowledge and skills by completing mandatory training, including SOAR.
  • To undertake any other reasonable task as requested by a supervisor or manager, which is within your competence.


  • A minimum of 12mths experience of working in a health and social care setting
  • Senior care/team leading experience
  • Care Certificate or equivalent training covering care induction as defined by Skills for Care.
